United States HMO, PPO, and Private Providers?

I am doing a research paper about health care in the United States and I’m a little confused about providers. I understand that if you have employment-based health coverage, you are sometimes able to choose between an HMO and a PPO and I understand the difference between these. What I am confused about is if you purchase coverage directly from a private provider. Do HMO’s and PPO’s still come into play, or is there a different system if you purchase insurance yourself? Any input is appreciated, thank you!
